Remote Therapeutic Monitoring Supporting Documentation

The following is a guide for documentation requirements with examples. This Document is broken up into two sections. The first section is the documentation requirements that the Supervising Physical Therapist must provide. The second section is the documentation that the MovementRx system
automatically produces and stores which you can access at any time.

Section 1: (Supervising Therapist Documentation Requirements)

Remote Therapeutic Monitoring Documentation for 98975 (Device Setup Once Per Patient)

• Document the type of device being used to monitor the respiratory and/or musculoskeletal system
• Document whether you provided the medical device to the patient or did the patient have their own medical device
• Document the education and/or training the therapist or assistant provided to the patient and/or their caregiver regarding the set-up and use of the
  medical device
• Document the education and/or training the therapist or assistant provided to the patient and/or their caregiver regarding what data to input, how to
  input the data and frequency to input the data

Example: In the Plan section of the initial evaluation and subsequent progress notes: This patient has been setup and educated on the use of MovementRx system for their home exercise program and the patient will be remotely monitored for the time they are in the clinic and up to 30 days following their last visit to ensure independence with their home program.

Documentation for 98980 and 98981

• Document changes to the POC as a result of the data analyzed and/or patient/caregiver interactive communication(s)

Section 2: MovementRx Generated Documentation

Documentation for 98976 and 98977 (RTM device to monitor MSK system status, each 30 days)

• Document the name and description of the medical device being used to monitor the respiratory and/or musculoskeletal system
• Document the number of recording(s) and/or programmed alert(s) transmission for the 30-day calendar period
• Document the data that was gathered by the medical device

Documentation for 98980 and 98981

• Document the date and minutes that the therapist or assistant analyzed the transmitted data
• Document the date and minutes of each interactive communication with the patient and/or caregiver and what was discussed during each interactive communication

Interactive Communication for 98980 and 98981

• It’s our understanding that interactive communication can occur via the following ways:

1. Phone call
2. 2-way audio-visual communication (FaceTime)
3. In-person

• Texting and email communication would not be interactive
