RTM Billing Monthly Report Instructions
The following is a guide for documentation requirements with examples. This Document is broken up into two sections. The first section is the documentation requirements that the Supervising Physical Therapist must provide. The second section is the documentation that the MovementRx system
automatically produces and stores which you can access at any time.
Remote Therapeutic Monitoring Documentation for 98975 (Device Setup Once Per Patient)
Example: In the Plan section of the initial evaluation and subsequent progress notes: This patient has been setup and educated on the use of MovementRx system for their home exercise program and the patient will be remotely monitored for the time they are in the clinic and up to 30 days following their last visit to ensure independence with their home program.
